The Scantest-815 is a compact, 1-sided quiz form measuring 3-3/8" x 6", designed for quizzes with up to 15 multiple-choice questions and five response choices (A-E). It includes a subjective score area and a bar code for easy tracking. This form is compatible with 888P, 888P+, and Scantron Score scanners.
